GAPS - Guidelines for Adolescent Preventive Services

Vulnerabilities
This questionnaire screens for some of the common health risks of teenagers, including drugs and alcohol and safety.

2 versions, covering 13-18s.

With 72 questions, sub-divided into sections about health, school, family, friends, relationships, aspirations.

With a parent questionnaire also.

Not validated, but used in clinical practice.

A tool from the USA.

Some amendments to the language may be needed but it has some useful screening questions for monitoring risky activity related to health (eg carrying a knife, wearing a bike helmet).
